Millesimo counts its birthday as 9 November 1206, when the Del Carretto marquises walled the village and castle beside the Gaietta bridge over the Bormida, a crossing that earned them tolls. The village lies on the river's right bank in the upper Val Bormida, and part of its land falls within the Bric Tana regional park.
